Person signing the consent form must be legal guardian.

If you have recently changed your name, update your records with the Social Security Administration before you apply for your license. The name assigned to the social security number must match the name that will appear on the Florida Driver License.

Church issued licenses cannot be accepted. Marriage license must be from a governmental agency. In some instances, more than one marriage license, divorce decree or court order document may be required to show the chain of name changes.

If the documentation submitted reflects any name change, an original or certified copy of your Marriage License, Divorce Decree or Court Order must be submitted.
